Liability Release Form & Code of Conduct
Between Voices of Carolina and Harvest Baptist Church of Rock Hill, further referred to as “[Organization]”
Located at 153 Miller Pond Road, Rock Hill, SC 29732
In consideration of participation by the undersigned in Voices of Carolina, the undersigned hereby releases, waives, discharges, and covenants not to sue the Voices of Carolina and Harvest Baptist Church, Kendra Husband, and any of its affiliates, employees, volunteers, and agents (including other practice or performance venues) from any responsibility or liability if any of the students or family members incur any injury, illness, death, damage to or loss of personal property while participating in the program or at the concert venues. We hereby release, hold harmless and indemnify the aforementioned against any and all liability costs and expenses arising in any way as a result of the undersigned family’s participation in Voices of Carolina. Any injury sustained on the Harvest Baptist Church (HBC)campus will be covered by the participant’s personal insurance policy. Any damage to the property will be personally reimbursed by the responsible parties. The undersigned does hereby agree to abide by all the rules and regulations that are presented by VOC and HBC. These guidelines and liability release applies to all activities of VOC including field trips, music festivals, and transportation to and from events away from HBC.
By signing this agreement, I acknowledge the contagious nature of the COVID-19 and voluntarily assume the risk that I or my family members may be exposed to or infected by COVID-19 by participation; and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by the COVID-19 while participating in the program may result from the actions, omissions, or negligence of myself and others, including, but not limited to, the VOC staff, church staff, volunteers, and program participants and their families. The undersigned agrees to be legally bound hereby and waive my right to maintain a lawsuit on behalf of myself or family members against the organizations listed above on the basis of any claim for negligence.
I authorize VOC and staff to use photographs, videos or quotes of any participants enrolled in choir for educational or promotional purposes of any type of media, including social media and its websites. I understand that I will not be paid or rewarded for providing this authorization.
Code of Conduct
In order to have a safe and enjoyable environment, all singers, parents, legal guardians, and other family members are expected to abide by the following guidelines when on the church property or in a choir activity.

I have read and understood the above guidelines and Code of Conduct. By signing below, I am agreeing to abide by these guidelines and any further directives given by the church and/or VOC choir leadership.
This contract shall be governed by the laws of the State of South Carolina in York County and any applicable Federal Law.
